Our commitment
Advanced Tech Hearing Aid Centers is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

- Health care operations. For quality review, staff training, scheduling, and normal business operation of our practice.
- Appointment reminders. To contact you about appointments, follow-up care, or device service.
- As required by law. When federal, state, or local law requires disclosure.
Other uses and disclosures — including most marketing and any sale of PHI — require your written authorization, which you may revoke at any time in writing.
Your rights
- Inspect and obtain a copy of your health record.
- Request a correction to information you believe is incorrect or incomplete.
- Request confidential communication at an alternate address or phone number.
- Request a restriction on certain uses and disclosures.
- Receive an accounting of certain disclosures we have made.
- Obtain a paper copy of this notice on request.
- Be notified if a breach occurs that may have compromised your information.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our office using the contact details below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
